Hyperprolactinemia And Erectile Dysfunction Hormonal And Oral Therapy

The presence of more than the normal levels of prolactin in blood is called Hyperprolactinemia. The normal levels in females are less than 580 mlU/L for women and less than 450 mlU/L for men. The increased prolactin level in blood is caused by the small benign tumors in pituitary part of the brain affecting ovulation and breast milk production. The hormone is down regulated by dopamine and is up regulated by estrogen.

Hyperprolactinemia results in decreased levels of testosterone in the blood resulting reduced erection causing erectile dysfunction in men. The level of testosterone is lowered also due to reduced secretion of dopamine. Bromocriptine a dopamine agonist is used to lower the elevated prolactin levels and it also reduce the size of the pituitary tumors in brain.

A study was conducted in a 67 year old man with Paget’s disease. The diagnosis shoed that there is a high level of prolactin in blood and the patient showed symptoms of erectile dysfunction. The erection is regained in Paget’s disease by normalizing the elevated prolactin levels by administrating bisphosphonates.

The main symptom in men with hyperprolactinemia is the loss of libido. This is due to the result of endocrine disorders resulting in either decreased libido or testicular atrophy in hyperprolactinemia patients. The pituitary tumors specifically prolactin secreting adenomas results in erectile dysfunction which is diagnosed by low serum testosterone level. Hyperprolactinemia induces the hypogonadism caused by the endocrinopathy in which the excess prolactin level in blood interferes with secretion of gonadotropin releasing hormone. This condition results in reduction in the level of testosterone which leads to loss of libido, inability to maintain erection resulting in erectile dysfunction.

Hyperprolactinemia which is caused by the pituitary tumor is treated by either using drugs like dopamine agonist or surgery will help to resolve the erectile dysfunction.
